Context

A private equity-backed pathology services platform had grown through multiple acquisitions, inheriting three laboratory information systems, two billing vendors, different report templates, and separate operating processes.

As case volume increased, collections per case declined. The company could see the overall performance gap, but could not reliably trace where revenue was being lost between a signed report, a billed claim, and a collected payment.

Specimens and stains documented in final reports were not always billed. Units could vary between laboratory and billing systems. Hospital directorship fees were sometimes missed, delayed, or billed at outdated rates.

The sponsor needed a clearer view of revenue performance, contract compliance, and quality-of-earnings risk across the network.

Assessment

IT Ally mapped the company’s complete case-to-cash process across finance, billing, laboratory operations, practice leadership, and pathologists. The team manually traced thousands of cases from accession through remittance to identify where revenue was being lost.

The assessment found recurring gaps across laboratory systems, billing files, payer payments, and hospital agreements, including:

  • Specimens and stains documented but not charged
  • Mismatches between billed and performed units
  • Documentation-related denials
  • Missed or under-billed hospital directorship fees
  • Limited visibility into newly acquired practices

IT Ally then built a proprietary case-to-cash platform in the company’s own cloud environment. The platform connected laboratory systems, billing vendors, electronic remittances, payer schedules, and hospital agreements into one centralized data model.

A configurable rules library standardized billable-case logic across the network. The AI layer reviewed reports alongside requisitions, specimen records, and claims to identify exceptions, classify root causes, generate addendum requests, and prepare underpayment appeal packages.

The company retained ownership of the source code, data model, rules library, AI prompts, documentation, and operating runbooks.

The Results

IT Ally delivered a fact-based revenue integrity platform that created measurable financial and operational value.

  • Recaptured recurring revenue equal to 2.1% of annual collections
  • Reduced unbilled stains and specimen-unit mismatches by more than 85%
  • Reduced documentation-related denials by 34% within two quarters
  • Recovered missed or under-billed directorship fees on one-third of hospital agreements
  • Reduced pathologist addendum requests by 40%
  • Reduced monthly revenue review preparation time by 60%

Newly acquired practices can now be benchmarked against the network within 30 days. The company also has a traceable, case-level revenue record that supports quality-of-earnings diligence and future exit planning.

The result was more than recovered revenue. The platform became a proprietary operating asset that improves visibility, supports scalable growth, and stays with the business through a future change of control.
